We are seeking a Nurse Case Manager to:

Join our team of talented case managers and social workers.
Have a broad clinical background with well developed assessment and critical thinking skills.
Understand medical necessity, level of care and patient status (inpatient vs outpatient).
Have some knowledge of community resources such as transportation, skilled nursing facilities or home health agencies.
Enjoy learning, asking questions, and working within a team setting. Education:

Minimum Bachelors degree in Nursing. Equivalent combination of education and experience will be considered.

Preferred Masters degree in Nursing or healthcare related field

Work Experience:

Minimum Three (3) years of relevant clinical nursing experience.

Preferred Recent experience as a hospital-based Nurse Case Manager.

Licenses and Certifications:

Minimum RN license in the state of Wisconsin.

Preferred ACMA certification as a Case Manager.

Required Skills, Knowledge, and Abilities:

Demonstrates ability for complex clinical decision making.
Uses critical thinking and problem solving skills effectively.
Excellent interpersonal communication and conflict resolution skills.
Computer skills in word processing, database management, and spreadsheet use. Physical Requirements:

Sedentary: Ability to lift up to 10 pounds maximum and occasionally lifting and/or carrying such articles as dockets, ledgers and small tools. Although a sedentary job is defined as one, which involves sitting, a certain amount of walking and standing is often necessary in carrying out job duties. Jobs are sedentary if walking and standing are required only occasionally and other sedentary criteria are met.

Work Schedule:

This is a Day shift position and includes eight (8) hour shifts, Monday - Friday; 8:00AM-4:30PM. Occasional weekend shift required. Hours may vary based on operational needs of department.
