The primary purpose of the Advanced Practice Registered Nurse (APRN), in the department Stem Cell Transplantation and Cellular Therapy, is to promote and maintain the health of SCT&CT patients across the care continuum. This position is required to work weekends per the staffing model.

Key Functions

Performs physical exams and interviews. Documents the review of systems (ROS) and physical exam, as well as the plan of care as decided by the care team. Discusses relevant findings with the physician. Ensures appropriate medications, blood products, and follow-up are ordered and scheduled. Teach patient/family about SCT/CT process, medical condition and follow-up. Uses critical thinking to manage care of blood and marrow transplantation patients.

Participates as team member by documenting, when appropriate ROS, physical exam and plan of care in a timely manner; coordinate and collaborates with other members such case managers, social worker, etc.

Actively participates in nursing organizations, keep self-current with certifications as determined by the department. Attends weekly departmental and bimonthly APRN meetings.

Demonstrates role and responsibility of SCT&CT APRN when presenting and educating new employees, nursing students and nursing.

Prepares patients for discharge by coordinating with receiving APRN/MD and ordering necessary tests and consults per SCT/CT standards of practice.

Inpatient responsibilities:

Conduct inpatient rounds with care team.

Assist completing SCT/CT flow sheet.

Order blood and platelets.

Document evaluations and submit billing on a timely basis per institutional policy.

Perform skin biopsy.

Complete toxicity and GVHD scale on flow sheet.

Follow-up on tests and consults.

Patient discharge teaching.

Schedule follow-up post discharge.

Discharge dictation summary during work week.

Work with nursing staff as a resource for team specific patients.

Schedule and infuse stem cells or product.

Manage complications of CVC.

Order appropriate restaging.

Co-manage pre-code situations.

Communicate discharge/care plan with multidisciplinary team.

Coordinate referrals/consults.

Complete progress note, when appropriate.

Assist with weekend APRN coverage.

Order appropriate medical interventions based on SCT/CT Standards of Care.

Outpatient responsibilities:

Evaluate patients in the outpatient settings.

Document evaluations and submit billing on a timely basis per institutional policy.

Assist completing SCT flow sheet.

Complete toxicity and GVHD scale on flow sheet.

Order medications, blood and platelets.

Perform skin biopsy.

Follow-up on tests and consults.

Schedule clinical studies as per guidelines and protocol.

Work with social work and nursing staff as a resource for team specific patients.

Schedule and infuse stem cells or product.

Manage complication of CVC.

Co-manage pre-code situations.

Communicate discharge to home care plan with multidisciplinary team and coordinate long term follow up.

Coordinate referrals/consults.

Order appropriate medical interventions based on SCT/CT Care Guidelines.

Assist with weekend APRN coverage.

Write or facilitate home health orders.

EDUCATION

Required:

Master's Degree in Nursing or Doctor of Nursing Practice. May be waived for applicants who obtained recognition as an Advanced Practice Registered Nurse from the Texas State Board of Nursing prior to 1/1/2003. Graduate education and related Texas Board of Nursing authorization must be consistent with clinical practice area and patient population.

EXPERIENCE

Preferred:

At least one year of experience in Advanced Practice Registered Nurse role.

LICENSURE / CERTIFICATION

Required:

All of the following:

Written authorization from the Texas Board of Nursing to function as an Advanced Practice Registered Nurse.

Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.

Pediatric Advanced Life Support (PALS), if working in the following departments: Pediatric - Patient Care (113100).

MAY BE REQUIRED: Authorization for limited prescriptive authority by the Texas Board of Nursing.
