FUNCTION:

Provide nursing care to Rainey Hospice House patients in accordance with medical directives, hospice care plan and interdisciplinary group meetings. Demonstrate and communicate the mission, ethics and goals of the facility as the face of Hospice. Professionally represent the organization with credibility, compassion and dignity and in the spirit of excellence and trust.

SUPERVISION RECEIVED:

The inpatient registered nurse works under minimal supervision of the charge nurse. The inpatient registered nurse and charge nurse report directly to the Patient Care Coordinator.

SUPERVISION EXERCISED:

The inpatient registered nurse supervises assigned licensed practical nurses and assigned certified nursing assistants in accordance to the regulatory policies/procedures.

EXAMPLES OF WORK PERFORMED:

These tasks are illustrative only; to carry out the day-to-day functions of the job, other duties may be performed. This position may require weekend, holiday and on-call work. Job Summary:

Provide holistic and patient/family centered care to dying patients and to grieving families in a hospice facility setting. Demonstrate clinical competence utilizing evidence based practices/research in all areas of the nursing process: assessment, planning, implementation, evaluation and outcomes. Incorporate knowledge of quality improvement and actively participate in the Quality Assessment and Performance Improvement Program (QAPI).

Be flexible, organized and work effectively and professionally under stress.

Knowledge of completing an admission with an admission note, competency to perform a complete physical assessment, develop and implement a nursing care plan for each assigned patient, perform an IDT (interdisciplinary team) care plan for each new admission and maintain knowledge of documenting via inpatient entry program.

Update the nursing plan based on changes in the patient's condition and communicate any changes to the charge nurse and Physician/Nurse Practitioner.

Ensure a smooth and safe hand off via shift to shift report to all related personnel (charge nurse, registered nurse or licensed practical nurse, certified nursing assistant and homecare nurses upon discharge of a homecare patient from the hospice house facility to homecare.

Adheres to all compliance and policy/procedure requirements of Hospice of the Upstate related to but not limited to:

Licensure renewal, regular attendance and punctuality is an essential function of all jobs at Hospice of the Upstate, medication administration, safety protocols, dress code policy, HIPPA, infection control and employee training (computer based, webinars, departmental educational classes, 75% required department staff meetings, 75% required Hospice of the Upstate staff meetings.

To practice within the scope of South Carolina Nurse Practice Act and exhibit knowledge of regulatory requirements as set forth by Medicare guidelines (CMS) and DHEC.
