*POSITION SUMMARY*
Provides primary clinical management to patients and families with Liver, kidney disease and abdominal organ transplantation including nursing care and delegated medical tasks. Works under the supervision and direction of a physician. Performs other duties as assigned.
*KEY RESPONSIBILITIES AND PERFORMANCE STANDARDS*
Responsible for the clinical management of patients in designated specialty following established protocols. Works with personnel to optimize care through chart review, case evaluation, development of clinical tools and protocols; acts as a liaison with the physicians and surgeons for resolution of patient issues.
Functions as a nurse practitioner for the designated population through performance of history and physical, development and documentation of plan of care, interpretation of test results, and revision of plan of care as indicated.
Orders/performs therapeutic/diagnostic studies/procedures as assigned through competency.
Evaluates outcomes of direct patient care and documents findings, interventions and results.
Provides the counseling and teaching for patients/families and other health care professionals related to their specialty area or role.
Performs other duties as required or requested, whether or not specifically listed on this job description.
Orders, alters or adjusts controlled and non-controlled substance medications for patients in assigned specialty, communicating with supervising physician.
Applies knowledge of growth and development in patient interactions and treatment.
Maintains activities for professional growth membership in appropriate professional organizations, attends at least one national meeting yearly, serves as a preceptor for nurse practitioner students, engages in professional writing.
Conducts or participates in research, special projects, or committee assignments related to role/specialty.
Exhibits a willingness to learn new tasks. Seeks the assistance of supervisor prior to beginning an unfamiliar assignment.

*Qualifications:*

*MINIMUM QUALIFICATIONS REQUIRED:*

* *Licensure/Certification/Registration:* Satisfies the requirements of the Louisiana Department of Health and Hospitals, Board of Nursing (Chapter 45). Current license as APRN in Louisiana.
* *Education: *Graduation from an approved Master of Nursing; Nurse Practitioner program.
* *Special qualifications:* Must be able to successfully complete established competencies for the position within designated probationary period. Current CPR certification required.
* *Hours of work:* varies as scheduled*.*
