Primary responsibilities of this faculty position include leadership of a statewide community-based clinical program entitled the Interprofessional Care Access Network (I-CAN), including educational, practice, and research activities; collaborate with OHSU Care Management programs as part of the Collaborative Advocacy through Community Healthcare (CATCH) initiative; teaching in the Portland Undergraduate Program. - I-CAN Program Director (0.20 FTE) providing statewide leadership, coordination, and communication with faculty, academic collaborators in OHSU health professions schools, and I-CAN partners in the community across the state
- Partner with the OHSU Care Management team on CATCH, an I-CAN related project (0.10 FTE funded by Hearst Grant)
- Coordinate Portland undergraduate faculty and students community-based clinical placements (0.10 FTE)
- Teach population health content in the undergraduate curriculum(0.40 FTE
- Participate in research and scholarship related to population health activities through I-CAN and CATCH
- Always promotes the mission of the School of Nursing through our Professional Standards of Customer Service, Reliability, Productivity, Professionalism, Accuracy, Timeliness, and Problem-Solving.
- Leadership & Project Coordination
- Lead the statewide I-CAN team and related activities.
- Expand innovative clinical experiences in community based settings for OHSU nursing students across the state.
- Liaison with all I-CAN partners, including community organizations, clinics, preceptors, lead faculty, and consultants.
- Coordinate community-based clinical faculty and Faculty in-Residence (FIRS).
- Lead program improvement activities.
- Project manager role for operations.
- Participates in CATCH with OHSU Care Management
- Supports FIRS to engage in and engages in neighborhood-community academic practice partnership (NCAPT) meetings.
- Teaching
- Teach population health content in the undergraduate program.
- Incorporate models into undergraduate programs and continuing professional development programming.
- Identify competencies, outcomes, and content to be included in a program of continuing professional development.
- Develop interactive modules that can be delivered by synchronous or asynchronous online methods.
- Clinical Coordination: Student
- Cultivate, facilitate and coordinate community-based clinical placements for community partner sites.
- Work with academic partners (SOD, Nutrition, SOM) to assign those students to community placements and facilitate interprofessional activities to meet learning outcomes.
- Assessment, Evaluation, and Scholarship
- Collaborates with evaluation team to develop and implement a program evaluation.
- Disseminate scholarly results from program and student assessment via presentations and publications.
- Review regional metrics and annual goals; identify baseline data.
- Collaborate with the OHSU Division of Care Management in overseeing and providing Hearst, OSBN, OHSU and other required evaluation and reporting.
- Seek out funding opportunities to expand and advance the I-CAN program.
- Master's degree in Nursing
- 5 years nursing experience
- RN licensure in the State of Oregon
- CPR - Basic Life Support - Health Provider Certification
- Prior experience in nursing clinical or academic education, population or community-based nursing.
- Ability to prioritize multiple tasks at one time.
- Must have excellent communication, analytical and organizational skills: both written and verbal.
- Ability to work independently and as part of a team while being collaborative in resolving problems.
- Must be proficient with computers running Windows and PC applications e.g. MS Excel, Oracle, Access, Work and PowerPoint.
- Must have demonstrated excellent customer service skills both on the phone and in person.
- Must possess energy and drive to coordinate multiple projects simultaneously.
- Ability to use tact and diplomacy to maintain effective working relationships.
- PhD in Nursing or related field or Doctor of Nursing Practice.
- 3 years' experience in nursing education and/or 3 years in public or population-based health, case management or primary care nursing.
- Must be able to travel to partner sites in Portland metro area and across Oregon
- Flexibility in schedule to accommodate the needs of clinical partners, students
- May include weekend or evening scheduling.
- Must be able to travel.
- Must be able to lift 20 pounds.
- Heavy use of computers, phones, and copiers.
- Ability to sit for extended periods of time.
