SUMMARY OF POSITION:

**$5,000 SIGN ON BONUS**

Responsible for providing routine nursing care under supervision to meet patient and provider needs within the practice site. Patient care includes accurately recording vital signs, discussion health concerns, performing testing, administering injections; assisting with procedures and documentation.

MINIMUM QUALIFICATION(S):

High School Diploma or equivalent required.

One (1) year of LPN experience.

Graduate of an approved school of nursing with current license to practice in the state of employment or eligible for licensure in the state of employment required.

Basic Life Support (BLS).

PREFERRED QUALIFICATION(S):

One (1) year experience in an acute or ambulatory care setting preferred
