Summary:

The Institute for Family Health is seeking Family Nurse Practitioners for its School-Based Health Centers in Manhattan & the Bronx. The Nurse Practitioner will report to the School-Based Health Program Medical Director.The Institute for Family Health operates SBHCs at two high schools in Union Square and Chelsea, an elementary/middle school in Harlem and two K-12 charter schools with Harlem Children's Zone. Each site operates from 8:30am to 4:30pm Monday through Friday.

Requirements:

Provide high quality urgent and primary care to children and adolescent patients enrolled in the School Based Health Center.
Provide appropriate specialist referrals, follow-up and hospitalization when indicated.
Share in the responsibility of site administration with other members of the School-Based Health team.
Establish strong partnerships with school administration and staff, community partners and School-Based Health support team.
Collaborate with other members of the School-Based Health team to coordinate the provision of health and wellness programming and education.
Ensure high quality care is delivered consistently and cost-effectively
Participate in site and program level quality improvement projects.
Maintain familiarity with ongoing research and development in relevant fields.
Maintain electronic medical records in compliance with Institute for Family Health standards with high efficiency.
Act as a preceptor for occasional students and staff.
When school is not in session, the SBHC NP will provide care at one of the Institute's two Family Practice hub sites in Union Square or Harlem.
Furlough for summer and school breaks is an option based on the position.

Qualifications:

Pediatric or Family Nurse Practitioner licensed in NYS prior to start date
Board Certified
Minimum of 1-2 years NP experience preferred
Proficiency in Spanish preferred
Comfortable working in a fast-paced team-based care environment
Excellent judgment and ability to prioritize resources where they will be most useful
Demonstrated ability to provide patient-centered care to children and/or teens
Outstanding clinical capabilities and judgment
Proficient in EHR systems, EPIC is used by the Institute for Family Health
Comfortable receiving and providing constructive feedback
Flexible and independent
Outstanding written and verbal communications skills with a culturally diverse patient and staff population
Passionate about providing high quality health care to low-income, under-served children and teens.
