Under supervision, the class works primarily in the field, providing professional nursing care to individuals, groups and families to improve the overall health of communities, or functions as a nursing team leader in a health clinic; and performs related duties as required

Essential Duties for Tuberculosis

Under the supervision of a PHN III, the PHN II works as part of an interdisciplinary team to provide case management for individuals on treatment for active TB disease, which includes performing in-home visits, providing education on tuberculosis and its treatment, checking medication regimens for appropriateness and monitoring for adverse effects, tracking lab results, identifying barriers to treatment completion, linking patients to pertinent community resources, communicating with treating clinicians, and participating in monthly case conferences. Tuberculosis nurse case managers also provide directly observed therapy, place and read TB skin tests, and assist with large on-site screening events (e.g., at a school or worksite).

Additional duties may be required for this position.

Hours: 8:00 am to 4:00 pm Days: Mon-Fri

Some additional hours may be required in case of a public health emergency

THIS POSITION IS IN THE CAREER SERVICE

Qualifications:

MINIMUM QUALIFICATIONS

Graduation from an accredited college or university with a bachelor's degree in nursing accredited by the National League for Nursing (NLN) or an approved foreign credential evaluation, plus one year of professional nursing experience.

Licensure, Certification, or Other Qualifications:

Must be licensed to practice as a Registered Nurse. Applicants must possess a valid State of Illinois Registered Nurse license at the time of employment.

SELECTION REQUIREMENTS

This position requires applicants to complete an interview which will include a written exercise and/or a skills assessment test as part of the interview. The interviewed candidate(s) possessing the qualifications best suited to fulfill the responsibilities of the position, based on the oral and written parts of the interview will be selected.

Preference will be given to candidates possessing the following:
  • Previous work experience in a healthcare setting with underserved populations.
  • Intermediate proficiency with Excel and Word.
  • Previous work experience in a clinical role within an acute care setting.
  • Previous experience working in the community.
  • Language proficiency in Spanish, Cantonese, or Mandarin.
